Creating quality guitars never meant for the masses

Meet the Builder

Richard Schlenk MD

For nearly 25 years I’ve practiced neurosurgery with precision, patience, and compassion. Along the way, I found a second calling: building guitars. This craft lets me channel those same values into something deeply personal and expressive. From word of mouth, I’ve built over 60 charity guitars—and I’m now offering limited commissions through Underground Guitars.

Guitar making, for me, is more than wood and strings. It’s about shaping instruments that inspire, comfort, and heal—just like the care I’ve given patients. Each guitar is crafted with respect for the musician who will wield it, play it, and make it their own. The goal is connection: maker to player, and player to instrument.

The process is collaborative. Musicians influence design while I preserve room for artistic freedom. I provide frequent updates and invite hands-on involvement, from shaping the neck profile to fine-tuning ergonomics. The result is a transparent, personal experience tailored from start to finish.

I specialize in original designs built from premium, highly figured woods and top-tier components. While every project requires real investment, the pricing stays fair for the materials, time, and artistry involved.

These aren’t mass-produced guitars. They’re instruments for players who want something defiant, unmistakably theirs, and absolutely alive in their hands.

Underground Guitars

From the shadows

Underground Guitars doesn’t advertise, doesn’t chase attention on social media, and doesn’t play by the rules. There’s no catalog, no sales pitch—only a glimpse of what’s possible. If you’ve found this site, it’s either by chance or someone’s direction. These guitars move by whispers, spoken of by those who’ve played them, built for those who seek something rare, personal, bold, and uncompromisingly different. Each instrument is crafted to break from the expected, to bend boundaries rather than follow them.
